Application Checklist

After you submit your application to an OASIS Transition Program, make sure you also complete these other important tasks as you prepare to begin your academic career at UC San Diego.

2017 Deadlines and Important Notes

  • If you are a freshman, we encourage you to apply to both of our transitions programs. Read through all of these instructions before applying for any of our OASIS Transition Programs. Please note that the applications to both programs are very similar, therefore be sure to save your essay responses on a word document so you can copy and paste for both.
  • You may use the links below to apply to each program.  After you've submitted your application, you won't be able to log in to apply to additional programs, so consider your options carefully.
  • Be sure to include your essay responses. Incomplete applications will not be accepted!
  • If you are applying to TRIO SSSP don't forget to mail or fax your income verification (1040 tax forms).
  • Application deadlines are 11:59 p.m. (PST):
    • Freshmen: Saturday, June 17, 2017
    • Transfers: Wednesday, July 5, 2017
  • You'll be notified about acceptance beginning:
    • Summer Bridge: Wednesday, July 5, 2017
    • TRIO Freshmen: Wednesday, July 5, 2017
    • TRIO Transfers: Friday, July 14, 2017

Transcripts & Test Scores

Transcripts

You can attend an OASIS transition program only if your final transcripts have been validated by the UCSD Office of Admissions and Relations with Schools. The validation process confirms that all admissions provisions have been cleared.

You must submit official transcripts from all high schools and colleges attended before the deadline of July 1. Begin requesting transcripts around May 1 or upon graduation (admitted freshmen) or around June 1 (admitted transfer students).

Freshmen should submit only their FINAL high school transcripts with graduation date (do not send 6th and 7th grade semester reports). If you attended more than one school, UC San Diego requires a transcript from each school showing the date of withdrawal or graduation. Also send any college transcripts reflecting college coursework no later than the postmark deadline of July 1.  Even if your transcript reflects college classes you took, you must request transcripts from each college you attended.

Transfer students must submit transcripts from all colleges and high schools attended.

Students with any transcripts from schools outside the United States must have all official transcripts sent before July 1.

UC San Diego also accepts electronic transcripts sent by the following e-transcript vendors: Docufide/Parchment, Credential Solutions and eScripSafe.

If your school has its own transcript request process, please follow that protocol and be sure you provide the correct mailing address for UC San Diego:

Test Scores

Freshmen applicants must request all official test scores from the testing agencies. These should have been received by UC San Diego in January. Please do not send any other documents or other information unless requested. If the UC San Diego Office of Admissions and Relations with Schools office needs more information, they will contact you directly by email.

Log into MyApplication and scroll down to the bottom of "My Checklist" where you will see TRANSCRIPTS AND TEST SCORES. Review "Official Test Scores" to see if the UC San Diego Office of Admissions and Relations with Schools has received them. If not, please make certain you requested them from the appropriate agencies.

When requesting test scores, specify all prior years taken; otherwise, only the latest scores will be sent. Please verify with your testing agency that your test scores have been sent to the UC San Diego Office of Admissions and Relations with Schools on your behalf. The College Board school code for UC San Diego is 4836.

Official ACT/SAT test scores were due in January from all freshmen applicants.

Test scores are processed in the order they are received by the Admissions office. You will need to allow a minimum of two weeks after scores are received for them to display on MyApplication. If you do not see receipt of your test scores after that time, notify the UC San Diego Office of Admissions and Relations with Schools office by email at receiptoftestscore@ad.ucsd.eduFinal test scores are due no later than July 15 (UCSD school code 004836).

For freshmen

Freshmen applicants must notify the UC San Diego Office of Admissions and Relations with Schools of any of the following:

  • changes to planned coursework in "a–g" courses for senior year
  • unsatisfactory grades such as a D or F received during senior year of high school
  • senior year weighted GPA falling below 3.0
  • changing schools
  • failure to meet the terms of the provisional contract after being offered admission as a freshman

You may not be able to attend Summer Bridge or be able to register for classes until your test scores are received and processed by the UC San Diego Office of Admissions and Relations with Schools. 

We reserve the right to revoke admission to Summer Bridge if you fail to fulfill these requirements. 

Immunization Requirements

Incoming Health Requirements – Deadline: July 15th, 2017

The health of the individual can affect the health of the campus community, and UCSD is committed to protecting the well-being of all our students.  All incoming students are required to meet the UC Immunization and Tuberculosis (TB) Screening Requirements.

Please attend to these requirements as soon as possible to avoid a hold being placed on your account which will prevent you from enrolling in classes.

Tuberculosis Screening Requirement

  • Enter the student health portal shs.ucsd.edu, click on "TB Requirement". Answer the five screening questions.
  • If you are required to do further testing, an additional form (TB Screening Admission Requirement) will be provided to download. If you are not required to do further testing, this is the end of the process!

Immunization Requirements

  • Log into your student health portal and click on "Immunization Requirement" and enter your immunization information into the online Immunization Record, then upload OR Fax 1-858-246-2414 your signed form  so we can verify your record.

  • Ensure your UCSD email accounts are active to receive a secure response to your question
  • Please go to “Profile” in your student health portal shs.ucsd.edu and enter your cell phone number and carrier to receive txt alerts

Health Insurance

It is strongly recommended that all OASIS Transition Program students acquire health insurance available for use in San Diego before entering the summer program.
Students who attend a program without health insurance will do so at their own expense. If you have health insurance, you'll be required to show valid proof upon entering the program.

Entry Level Writing Requirement

If you have not yet satisfied the entry level writing requirement (ELWR), please visit awp.ucsd.edu for more information.

Math Placement Exam

You must have some type of math placement test results in order to enroll in any UC San Diego math course. All students who apply should take the MPE by June 17, 2017. (see FAQs for details)

College Orientation

Make sure to sign up for and attend your college orientation — the sooner, the better!

In addition to signing up for orientation, make sure to complete all additional information required by your college.

Enroll in Fall Quarter Classes (End of August)

Enrollment/registration for fall quarter classes takes place at the end of August.

In order for your enrollment process to go as smoothly as possible, it is essential that you take care of all the above (e.g., immunization requirements, college requirements, transcripts, etc.).

Your Student E-mail Account

Begin to check your UC San Diego e-mail account and continue to check it regularly. This is the official mode of communication at UC San Diego.
